CMS released a transmittal (R3P247i) updating the Home Health Agency Cost Report, Form CMS-1728-20, by clarifying and correcting the existing instructions, forms, and electronic cost report (ECR) specifications. These changes will impact cost reporting periods ending on or after August 31, 2022. Changes include:

  • A general edit clarifying instructions regarding rounding standards for fractional computations:
    • 1045S The CBSA count on Worksheet S-3, Part III, column 1, line 34, must be greater than zero. [09/30/2021] pg. 47-349
    • 1280S Worksheet S-3, Part V, column 3, sum of lines 4 through 13, and 17 through 26, must be greater than zero. [08/31/2022] pg. 47-351
  • Correcting line number in the instruction column for recommended sequence for completing HHA cost reporting form
    • Worksheet D-1, column 2, line 4, and the sum of Worksheet D, Part II, column 1, lines 10 through 15 and lines 17 through 21, minus line 16, must both be greater than zero. [12/31/2020]. pg. 47-353
  • Updating Worksheet D, Part II
    • Clarified instruction for line 16 to include Other Adjustments as found on the PS&R.
    • Corrected the calculation of the subtotal line on line 23.
    • Revised the instructions for lines 32 and 32.75 in accordance with ยง2 of the Protecting Medicare and American Farmers from Sequester Cuts Act of 2021 (PAMA), applying a sequestration adjustment of 1 percent from April 1, 2022, through June 30, 2022; and 2 percent on or after July 1, 2022.
  • Redesignating edit 2100S as 1225S
    • 1225S Total visits for the HHA in Worksheet S-3, Part I, column 7, line 11 and unduplicated census count for the HHA in Worksheet S-3, Part I, column 8, line 13, should be greater than zero. [12/31/2020] This edit was previously 2100S. pg. 47-350
