Tallgrass Creek has been  recognized as one of the Top 10 Skilled Nursing and Rehab Facilities in the Kansas City Area by the National Post-Acute Continuum. This honor is based on CMS Quality Rating and Financial Efficiency Scores.

NPACC cited Tallgrass Creek, which is managed by Erickson Living, May 25th at its Readmissions and Care Transitions Summit in Overland Park.

Tallgrass Creek was specifically honored for its CMS 5 Star Quality rating, which is the highest rating given by Medicare. It was also recognized for its hospital readmission rate for all Skilled Nursing guests within 30 days of discharge (6.6% versus the National Average of 17.5%).

The National Post-Acute Care Continuum (NPACC) is a non-profit, 501 (c)(3) professional education and networking resource. Its mission is to support improving and best practices through productive collaboration, continuing education and effective event coordination at the national and local levels.

The purpose of NPACC’s national summits and local chapter events is to showcase patient-centered, best-practice integration models that assist providers and hospital systems in meeting the mandates of The Affordable Care Act (ACA), and to offer a new platform for sharing them across the continuum.

As a non-profit organization, the NPACC can achieve more through collaborative action.
