At the recent Fall Policy & Leadership Conference, Kim Burnam, KDHE Director of Eligibility, discussed the importance of the Facilitator Form for members assisting residents with their Medicaid Application process. This form will allow members to talk to the Clearinghouse personnel about the resident’s application.

Please note the attached facilitator form contains the information below.  This will go beyond 6 months and can be up to 12 months.  This will allow members to receive notices that would normally just go to the family.  This will help with tracking the application.

I understand that this authorization will expire 6 months from the date this form is signed or once my application is completed, whichever is later. I choose to provide a different date for the expiration of this release: _____________.   An appointment of a community organization, a medical provider, or staff cannot exceed 12 months.
