Don’t miss the next session of our Value Based Care Virtual Learning Series on Frail Elder Physician Practices, at 1 PM on May 27.

This module will explore the growing field of frail elder physician practices and primary care groups that focus on homebound individuals and residents of ALFs and SNFs. Participants will learn how ACO expansion has accelerated the growth of these practices, how their economic models differ from traditional clinic based geriatrics, and how these groups influence SNF financial and quality outcomes.

This session is part of our member-benefit Value Based Care Learning Series and is available at no cost to members. If you or someone one your team is interested in this or other sessions in the series, please register here.
