The KanCare MCOS are in the midst of completing their state-required health risk assessments and person-centered plans for their Members who are nursing home residents. Until this week Sunflower had been requesting significant assistance and information form nursing homes to fulfill their requirements.  Sunflower will now complete a short Person Centered Plan, as do United HealthCare and Aetna Better Health. When completing the assessment and person-centered plan, KanCare MCOs will work with the Member resident and his/her DPOA or legal representative as much as possible to get the required information, and to reduce the work of nursing home staff.
