Anxiety is one of the leading causes of hospital readmissions and crosses many diagnoses. The Anxiety Shared Pathway Toolkit focuses on using integrative health care to help treat anxiety. Integrative health care emphasizes a holistic, patient-focused approach to health care and wellness – often including mental, emotional, functional, spiritual, social, and community aspects – and treating the whole person rather than one physical symptom. It aims for well-coordinated care between different providers and settings of care.  hank you to the Lake Superior Quality Innovation Network for developing and sharing this excellent resource.
