Insights from a Fellow Provider: Medicaid Applications and Renewals for Veterans

A fellow provider ran into some issues with a Medicaid application and renewal for a veteran. After some back and forth, these are the details they were able to figure out.

Medicaid applications for those who are veterans must have form ES-3122 submitted to go with their Medicaid application. It is in Medicaid policy that this form must be completed, or Medicaid may be denied.  Providers need to reach out to their closest veteran service representative and ask them to make a visit to the facility to visit the veteran or try to make an appointment at their office if they can do so.
