A recent question from a member brought up the subject of Advance Beneficiary and Notice of Medicare Non-Coverage Forms. Surveyors are known to check for the correct use of these forms and that the most current form is being used. It is a good practice to have in your policies that someone in the organization is checking periodically (at least quarterly is good) that you are using the latest form.
