State-licensed Home Plus, Assisted Living, and Residential Healthcare providers:

July involuntary discharge reports are due no later than 5:00 p.m. on August 31, 2026.

All Providers Must Report — Even Zero Discharges

Every provider must submit a report each month, even if the facility had zero (0) involuntary discharges during the reporting period. Reporting zero is important to help establish an accurate statewide picture of how frequently involuntary discharges are actually occurring.

Don’t Miss the Deadline

Providers that do not submit their July report by the August 31 deadline will receive a warning letter directing them to complete their reporting as soon as possible. The warning is a one-time opportunity. Providers that fail to report after receiving the initial warning may be subject to a state civil money penalty (CMP) for noncompliance.

Providers should be receiving email updates and reminders through KOTA, and KOTA is also where the required reporting must be completed.

Bottom line:If you had zero discharges, you still need to report. If you had discharges, report those as well. All providers must submit by 5:00 p.m. August 31.
