New long-term care focused Drug Diversion Prevention Pathway and support tools to help you increase awareness, assess your processes, create individual plans for intervention implementation, and measure your outcomes. It includes leadership guidance and education, curriculum and competency, equipment guidance, and draft letters to delivery services, long-term care medical directors and long-term care pharmacists. The resource is a joint project between the Minnesota Department of Health and Minnesota Department of Human Services, which sought input from a variety of partners that included LeadingAge Minnesota.
