Residents rely on you to maintain and increase their quality of life. Providing timely interventions is vital to prevention of a significant change. ADL function is now part of the 5 Star rating system and CMS is putting more pressure on homes to prevent significant change in condition. Each home is invited to bring the DON, a charge nurse, restorative aide, therapist, CNA, and/or social services/activities staff to this training. In addition, one resident’s family member is required to be a part of the team. Families are vital in recognizing a change in condition. The team you choose will learn information and be provided in-service materials to train other staff and family in your organization. Following the workshop, each organization will provide a summary of the in-service provided to their staff and families as part of the grant expectations.
