The Civil Money Penalty Reinvestment Program (CMPRP) would like to make you aware of the Breakthrough Community on Adverse Events, an exciting new program launched by the Centers for Medicare & Medicaid Services (CMS) designed to improve the quality of life and quality of care for nursing home residents.

The Breakthrough Community on Adverse Events is a voluntary opportunity for nursing homes to work collaboratively to test interventions that will lead to improvement in quality of life and quality of care for residents. The Breakthrough Community on Adverse Events is expected to last about one year. The CMPRP team believes that the participation of your nursing home will strengthen your ability to focus on quality improvement through a structured, evidence-based process. We believe the peer-to-peer model will greatly benefit nursing homes in your area through the opportunity to share and learn about quality improvement efforts and potentially support quality improvement in nursing homes nationwide. How to be part of a Breakthrough Community

To apply to be part of a Breakthrough Community, complete the application and email it to CMP-info@cms.hhs.gov. To apply, applicants must:

  • Be a nursing home located in Kansas and Nebraska.
  • Ensure full participation of a core team in the Breakthrough Community, including attending all learning events, as well as monthly calls or webinars.
