Tips for Prescribing in the Nursing Home, by L. Liu and I. Campbell. Volume 24 – Issue 9 – October 2016 – ALTC.

Nursing home residents are at increased risk of adverse drug reactions, falls, hospitalization, and mortality due to polypharmacy. It can be a challenge, however, to know when, how, and which medications to discontinue. In order to help guide clinicians with the deprescribing process, the authors provide a tip sheet which includes a detailed step-by-step guide on how to safely deprescribe as well as accompanying tables to help clinicians identify medications that may be potentially harmful or contribute to polypharmacy.
