Researchers from the NYU College of Nursing are testing a survey IN-HOME (Identifying Needs to Help Older adults Maintain Everyday community living) in a diverse nationwide sample of 150 persons living with dementia (PLWD) who meet eligibility criteria and 150 family care partners (FCPs). The NYU research team would go to the nursing home sites to complete the survey in person with the residents and conduct the FCP surveys virtually/via phone.
The goal of the study is to identify supportive measures to help older adults maintain everyday community living. The survey will also help prepare nursing homes for CMS’ 2027 Discharge to Community—Post-Acute Care (DTC-PAC) Measures that will assess successful discharges of residents not being admitted to a hospital after being discharged from the Skilled Nursing Facility.
