CMS released two new tools to help home health agency teams improve their Quality Assessment and Performance Improvement (QAPI) programs for the launch of the expanded Home Health Value-Based Purchasing Model in January. First there is brief, self-guided, performance improvement activity workbook is based on structured review of the HHVBP performance data by measure category. The forms included in the workbook create simple data visualizations that will allow teams to see patterns in measure performance by category. Teams can then self-assess their agency’s performance by measure category for use in planning performance improvement activities.
